On 16 September 2026 Reuters reported OpenAI's plan for regular public reports on unexpected behaviour, after scrutiny of a test-time breach at Hugging Face. Public disclosure by a vendor does not assign an owner inside your organisation. If a system can act and no named role can suspend it, the exposure is already there, whether or not an incident has been written up. Hidden exposure is the default when authority is granted in Slack and never put in a record.
